This document outlines an April 7, 2026 educational webinar on the forthcoming <strong>2026 Surviving Sepsis Campaign (SSC) Adult Guidelines</strong> and highlights how the update was developed and what is new versus unchanged. Methodologically, the update followed standards for trustworthy clinical practice guidelines using <strong>PICO questions</strong>, systematic literature searches, evidence synthesis (including meta-analysis and risk-of-bias assessment), and <strong>GRADE</strong> to rate certainty of evidence. Recommendations were shaped through an <strong>Evidence-to-Decision (EtD) framework</strong> considering benefits/harms, values, resources, equity, acceptability, and feasibility. Draft recommendations were debated in person (March 2025) and finalized through multiple voting rounds with high participation and agreement. Key content highlights include: - <strong>Screening/Early management:</strong> <em>Insufficient evidence</em> to recommend <strong>novel rapid host diagnostics</strong>. - <strong>Antibiotic management:</strong> <strong>Timing remains the same as 2021</strong>, while <strong>antibiotic selection places greater emphasis on stewardship</strong>. - Several statements were <strong>upgraded from conditional to strong recommendations</strong>, including areas informed by newer evidence (e.g., a 2024 JAMA meta-analysis on prolonged vs intermittent β-lactam infusions). - Added attention to <strong>therapeutic drug monitoring</strong> (balancing efficacy and toxicity) and <strong>source control</strong>. - <strong>Hemodynamic monitoring/resuscitation</strong> evidence certainty was upgraded in parts (e.g., low→moderate; very low→low), and multiple “**In Our Practice**” statements address practical issues such as peripheral vasopressors, vasopressin/epinephrine add-on criteria, SpO₂ targets, and corticosteroid use. The SSC website will provide <strong>new tools</strong> to support guideline implementation.
