In this video, Dr. Nikhil Jagan discusses the concept of awake self-proning in COVID-19 patients with acute respiratory distress. Prone positioning, or placing patients on their stomach, has been shown to improve mortality in intubated patients with ARDS. Awake self-proning is being explored as a potential alternative to invasive mechanical ventilation, particularly in resource-limited settings. Some studies have shown that awake self-proning can improve oxygenation and reduce intubation rates and mortality in non-intubated COVID-19 patients. However, the ability to tolerate prone positioning can vary. The NIH recommends considering awake prone positioning for patients with persistent hypoxemia, but not as a rescue therapy for refractory hypoxemia. Ongoing studies aim to further evaluate the safety and effectiveness of awake prone positioning in COVID-19 patients.
