Dr. Heidi Smith presented on the assessment, prevention, and management of delirium according to the 2022 SECM PEDS PANDEM Clinical Guidelines. The prevalence of delirium in pediatric patients has become better understood with the use of specific tools. It is important to be aware of the risk factors associated with ICU delirium in children, such as younger age and baseline neurodevelopmental delay. Delirium is associated with longer hospital stays and increased costs. The association between delirium and mortality is not clear but requires vigilance. The PANDEM Guidelines recommend using validated delirium screening tools, such as the Corneal Assessment for Pediatric Delirium (CAPD) and the Pediatric and Preschool Confusion Assessment Methods for the ICU (PACAM-ICU), for routine monitoring. Non-pharmacologic strategies, such as optimizing sleep, interdisciplinary rounds, family engagement, and early mobility, are suggested for prevention. While there is limited evidence for the use of haloperidol or atypical antipsychotics, they may be considered for severe delirium manifestations. Minimizing benzodiazepine-based sedation is recommended to decrease the incidence and severity of delirium.
