The speaker discusses the importance of nutrition therapy in pediatric patients with sepsis. Malnutrition is common in these patients and is associated with worse outcomes. Adequate enteral nutrition (EN) has been shown to decrease complications and mortality. Guidelines suggest early initiation of EN within 48 hours of admission, even in patients on vasopressors. Gastric feeding is preferred, and routine measurement of gastric residual volumes or use of prokinetic agents is discouraged. However, these guidelines are based on weak evidence. Feasibility studies are needed to address barriers to successful delivery of EN in this patient population. An adult study looked at trophic EN compared to no EN in patients in septic shock on vasopressors and found that trophic EN was feasible and associated with increased vent-free and ICU-free days. More research is needed to guide nutrition therapy in pediatric sepsis patients.

The updated pediatric sepsis guidelines were published in early 2020. Many recommendations were based on low quality of evidence, thus making some clinicians unsure of whether or when to employ them. Since early 2020, several studies have been ongoing that may help clarify some of the guideline recommendations, specifically in the areas of fluids, steroids/metabolic support, and parenteral nutrition. This session will provide synthesis of study results regarding these therapies and will help attendees understand whether and how these therapies are best utilized in the pediatric sepsis population.
