My name is Matt Shubin, I'm an intensivist at the Cleveland Clinic in Cleveland, Ohio. Today I'll be talking to you about mechanical ventilation training and social media, expanding  reach and maintaining quality. I have no conflicts of interest related to this talk. As we consider the current state of conventional ventilator education, we can see several areas  for improvement. A major issue with traditional educational platforms for mechanical ventilation education is that they are static.  It often involves reading a page with no interaction, with a static waveform that may or may not be representative of what we might see in real life.  The reach of traditional educational mechanical ventilation is usually small and limited to those who can fit inside of a classroom or lecture hall.  Furthermore, the majority of these educational platforms are trainee-focused, which leaves  out opportunities for lifelong learning unless they're specifically structured for continuing medical education.  Now that we've considered the limitations, let's think about the potential benefits for social media education compared to these conventional approaches.  Social media creates an opportunity for a worldwide reach in the educational materials  that are prepared, including low- and middle-income countries as well as high-resource settings. Social media education can be both dynamic and interactive in that you can let cases  evolve over time as well as interact with your audience in a variety of ways. A major benefit of social media education is the flattened hierarchy, which allows  debate and inquiry from a variety of learners in a variety of settings. These are things that may not happen in a live classroom where someone may be a little  bit less likely to speak up. Spaced repetition can occur throughout social media education, and it's whenever the learner wants it, especially for enduring materials such as videos.  These platforms also allow for dissemination of peer-reviewed publications in ways that we may not otherwise be exposed to them.  Next, we'll consider some examples across a variety of platforms. One thing that is in common for all of these platforms is that successful content tends  to be visual, brief, and interactive. Starting with YouTube, the most common uses for mechanical ventilation education include  pre-recorded lectures as well as archives of live discussions. In this video, we show an example of demonstrating flow starvation by use of a simple pegboard with some string.  The full video is seen at the link at the QR code which is attached.  Additionally, we can see a large number of curated content here from a SEVA event round series from the Cleveland Clinic, which is a bi-weekly series and includes hour-long  lectures with discussions, and again there's a QR code to follow up that content. Believe it or not, Instagram is another great platform you can use for social media education.  You can include video, picture, and infographic posts as well as stories with quizzes. Quizzes allow the learner to assess their understanding of the topic and as well allows  the content creator to see some metrics and understand where people are having difficulty understanding the topics. Here's an example of an Instagram story.  Teaching is interspersed with the quiz questions. The learner is given another opportunity to evaluate a live vent waveform and then we give the answer to the question.  We can then follow it up with how one might intervene in that situation.  Perhaps the most commonly used platform for mechanical ventilation education is Twitter. This allows for one-off waveform reviews which can be creator-generated or crowdsourced,  detailed discussions on strategies for difficult ventilation cases, as well as quizzes embedded in the Twitter application.  Here's an example of a prompt image with a large number of abnormalities in the waveform. The creator then asks the audience what they think might be going on in this case and after  some time allows for the answer to be given in full. Here we see the full answer provided and in this case we're actually able to show three  different types of trigger abnormalities. As mentioned prior, there's an opportunity also to somewhat crowdsource the information in order to create teaching cases.  As you can see, once again, there's a large amount of information in this image and many opportunities for teaching with one single piece of ventilator waveform data.  Social media blogs also allow for education with very different mediums, whether they be text, video, or in this case infographic, and there's a QR code to the link for the  full infographic in this case. Beyond just waveform education, these blog formats also allow for teaching of key pulmonary physiology topics such as transalveolar pressure.  There are other platforms available for dissemination of social media mechanical ventilation content that we haven't quite explored yet.  TikTok is a platform which could allow for ultra-brief presentations of more fundamental topics and live chats can happen across a number of platforms, including Twitter and  Instagram which we already discussed, but additionally Discord is another potential platform. A major concern in dissemination of social media education is quality and whether or  not it has undergone review. Let's review the current state. There are a few sites in the free open access medical education sphere which provide pre-publication  peer review and it's usually listed on their site with a specific reviewer. Furthermore, social media allows for real-time feedback if there is a mistake or if there  is a misunderstanding in the way the content was presented. It's important to stay vigilant when you see new content produced and view everything with a slightly skeptical eye.  It's important, though, to remember that most talks you hear at conferences and locally aren't peer-reviewed either.  The good news for social media, though, is the flattened hierarchy and rapid responses to content produced help course-correct very rapidly.  Revisions can be made in minutes compared to weeks or months when journals have to publish corrections.  What are the future opportunities for mechanical ventilation education in social media?  Ideally, we can move to a fully free, open access, and pre-publication peer-reviewed system, but this will take significant investment from other parties.  Peer-review and publication by professional societies and journals could go a long way in lending credibility and quality to the topics and content that are produced.  One great early example from ATS Scholar is their Ventilators for Non-Intensivists series, which is fully peer-reviewed, counts as a publication, and additionally is free and  open access. And the QR code there will take you to that particular video. If you're interested in learning more about mechanical ventilation education in social  media, here are some places where you might look next. Hashtags which are predominantly used on Twitter for mechanical ventilation focus on these  two areas, number one, hashtag VentRounds, and number two, hashtag SoMe4MV, which is social media for mechanical ventilation.  Links for more specific content that I have been involved with include the Cleveland Clinic SEVA VentRounds series, which occurs again on a bi-weekly basis, and the link to sign  up is there. At criticalcarenow.com, everything is pre-publication peer-reviewed, and we have a section for airway  and ventilators, as well as a respiratory care section, which is actually produced by respiratory therapists.  In closing, there are fantastic opportunities for educating learners in social media when it comes to mechanical ventilation.  As you can see, these benefits would also apply to many other areas within critical care education.  Most of the people that produce this content are doing this for free and in their free time.  Buy-in from societies and journals could support high-quality peer-reviewed content and perhaps support the creators themselves.
