Inter-ICU transfers refer to the movement of critically ill patients between different intensive care units. The exact prevalence of such transfers is not well known, but it is estimated that around 5% of critically ill patients are transferred between hospitals. Regionalization is a strategy that involves concentrating patients at specialized centers to improve outcomes and access to expertise. While regionalization has been successful in other areas of medicine, such as trauma care and stroke treatment, its effectiveness in critical care is still debated. One specific focus within critical care that could benefit from regionalization is acute respiratory failure. Patients with acute respiratory failure account for a significant portion of ICU admissions and often have poor outcomes. Studies have shown that high-volume centers have better outcomes for patients with acute respiratory failure, suggesting that regionalization could improve care for these patients. However, there are challenges with regionalization, such as potential delays in treatment during transfers and limited capacity at referral centers. Additionally, the impact of inter-ICU transfers on families is often overlooked. The process can cause significant stress and financial burden, and families are often not involved in the decision-making process. Studies have found that families of transferred patients experience higher levels of anxiety, depression, and post-traumatic stress. There is a need for improved communication and support for families during inter-ICU transfers. Ultimately, more research is needed to better understand the prevalence and impact of inter-ICU transfers, as well as the effectiveness of regionalization strategies. The goal is to improve outcomes for critically ill patients while also addressing the needs of their families.
