The Surviving Sepsis Campaign has released updated guidelines for the management of COVID-19. The guidelines were developed by a panel of over 50 experts from around the world and provide recommendations for the treatment of severe and critical cases of COVID-19. The guidelines use the GRADE approach and a rigorous methodology for developing recommendations. The panel formulated questions in a PICO format, conducted literature searches, assessed the quality of evidence, and utilized an evidence-to-decision framework to guide their recommendations. The panel addressed several key questions including the use of awake proning, corticosteroids, hydroxychloroquine, remdesivir, and venous thromboembolic prophylaxis. For awake proning, the panel found insufficient evidence to issue a recommendation for or against its use in non-intubated adults with severe COVID-19. For corticosteroids, the panel issued a strong recommendation favoring the use of systemic corticosteroids in severe or critical COVID-19. For hydroxychloroquine, the panel issued a strong recommendation against its use in severe or critical COVID-19. For remdesivir, the panel issued a weak recommendation for its use in severe COVID-19, and a weak recommendation against its use in critical COVID-19. For venous thromboembolic prophylaxis, the panel issued a strong recommendation for its use in severe and critical COVID-19. The guidelines will be updated as new evidence emerges.
