Dr. Karin Chung from McMaster University introduces the concept of ICU liberation in pediatric practice. ICU liberation, also known as the A2F bundle, is a series of evidence-based practices that aim to reduce ICU-related morbidities in critically ill children. These bundles, such as PICU-Liberate and Liberazione, are designed to deliver ICU-based rehabilitation consistently to every patient, every day. Rehabilitation is important in critical care because most pediatric patients now survive their critical illness, but often experience long-term physical, cognitive, psychosocial, and family-based issues. ICU liberation practices improve patient outcomes, reduce mortality and readmission rates, and improve the quality of care. However, these bundles are currently inconsistently applied in pediatric and adult ICUs, highlighting the need for better implementation. ICU liberation is complex and requires coordination from multiple interprofessional team members. Despite challenges, ICU liberation is rewarding and brings people together, benefiting both patients and families. Ultimately, sharing the concept and resources will lead to its improvement and permeation into practice.
