In a session on the year in review for the CPP section, three topics were discussed. The first topic was the use of opioid-sparing adjunct therapy for managing pain in critically ill patients. It was highlighted that the opioid epidemic has led to a reevaluation of prescribing practices and the need for alternative pain management strategies. Multimodal agent therapy, including non-opioid analgesics like Tylenol, ketamine, and neuropathic medications, was discussed as a potential option. The second topic focused on the management of hematology oncology critical care patients. The use of chimeric antigen receptor T-cells and immune checkpoint inhibitors in cancer treatment has led to the need for intensive care support. The challenges of managing toxicities associated with these therapies were discussed, along with the need for further research to optimize dosing, duration, and clinical outcomes. The third topic focused on emergency medicine, specifically the use of tranexamic acid for traumatic intracranial hemorrhage, the potential use of vasoactive agents for hemorrhage, and the treatment of status epilepticus. The findings from a study on tranexamic acid showed that it did not improve outcomes in traumatic brain injury patients. The use of vasoactive agents for hemorrhage showed promising results in reducing blood product requirements, but more research is needed. Finally, a study on the treatment of status epilepticus found that all second-line anti-epileptic drugs, including levotiracetam, fosfomycin, and valproate, had similar efficacy rates. The importance of adequate dosing and the need for further research in this area were discussed.
